Forms For The Class Of 2022
Spring – Junior Year (Due April 15, 2021)
- Parent Survey – A Guidance Counselor recommendation accompanies every copy of a school record sent to a college. We welcome parental input in helping portray the most accurate and positive picture of each student. By providing information on this form, you are consenting to also sharing this information on your Guidance Counselor letter of recommendation. Please do not include anything you would like to remain private.
- Student Survey – A Guidance Counselor recommendation accompanies every copy of a school record sent to a college. We welcome student input in helping portray the most accurate and positive picture of each student. By providing information on this form, you are consenting to also sharing this information on your Guidance Counselor Letter of Recommendation. Please do not include anything you would like to remain private.
- Teacher Recommendation Survey – This survey is a critical piece of information gathered to allow the teacher to create the most comprehensive academic picture of each student. The teacher will be utilizing the student’s input to compose the recommendation letter. The student must complete one survey for each teacher they wish to receive a letter of recommendation from.
Fall – Senior Year (Due Fall 2021)
- Transcript Request – Due to the large volume of applications processed by the College Counseling Office, students must submit their College Deadline Form to the College Counseling Office TWO WEEKS prior to any deadline date. Please note: The College Counseling Office will only send transcripts and letters of recommendation to the schools listed on this form.
- Teacher Recommendation Request Form – Due to the large volume of letters teachers are asked to write for their students, students must submit this recommendation form to their teacher TWO WEEKS prior to any deadline date. Teachers will only submit letters to the schools requested on this form.
- Authorization for Release of School Records – By submitting this form electronically, the parent/guardian authorizes the Guidance Department of Bergen Catholic High School to release their student’s transcript file to all the institutions and/or scholarship services designated by the student, and to any college requesting the information for athletic or admissions purposes. This form must be submitted TWO WEEKS prior to any deadline date. Please note that Bergen Catholic High School DOES NOT send standardized test scores. It is the student’s responsibility to request them from the testing agency.
